This can lead to issues of trust, potential for tampering, and difficulties in reconstructing the exact history of a patient's condition or treatment decisions. ChronosMed proposes a decentralized solution that creates a granular, time-stamped, and cryptographically secured audit trail for every significant interaction with a patient's medical record.
